Foundational Community Supports


Foundational Community Supports (FCS) is a program offering benefits for supportive housing and supported employment for Apple Health-eligible beneficiaries with complex needs. Through this program, we help clients find and maintain jobs; acquire stable, independent housing; and gain the necessary skills to be successful.


Supportive Housing services include help identifying housing resources, support in obtaining a lease, assistance in relations with landlords, and independent living skills development.

Who qualifies?

  • 18 or older

  • Medicaid-eligible

  • One assessed health need:

    • Mental health need

    • Need for outpatient SUD treatment 

    • Need for assistance with 3 or more activities of daily living (receiving long term care services)

    • Need for hands-on assistance with 1 or more activities of daily living (receiving long term care services)

    • Complex physical health need

  • One risk factor:

    • Homeless for at least 12 months or homeless on at least 4 separate occasions in the last 3 years (combined to equal at least 12 months)

    • Two or more stays in an institutional setting in the past 12 months

    • Two or more adult residential care stays in the past 12 months

    • Frequent turnover of in-home caregivers

    • PRISM score of 1.5 or above


Supported Employment services include vocational/job-related assessment, planning for employment, job placement, development, and coaching, and skills-building for negotiating with prospective employers.

Who qualifies?

  • 16 or older

  • Medicaid-eligible

  • One assessed health need:

    • Mental health need

    • Need for outpatient SUD treatment

    • Need for assistance with 3 or more activities of daily living

    • Need for hands-on assistance with 1 or more activities of daily living

    • Objective evidence of physical impairments

  • One risk factor:

    • Unable to be gainfully employed for at least 90 consecutive days due to mental or physical impairment

    • SUD with repeated inpatient treatment episodes

    • Diagnosed mental health and/or SUD that is at risk of deteriorating w/o intervention

    • Inability to obtain or maintain employment resulting from age, physical disability or TBI
